Winter 25/26 Season
High Bridge Middle School offers Boys and Girls Basketball and Cheer in grades 5-8 for the Winter season. To participate in Middle School sports, students must complete the following.
As of July 1, 2024, there is a new NJDOE Sports Physical Form and Preparticipation Physical Evaluation Medical Eligibility Form. You must bring the forms to your physician when your child receives their physical. The doctor will complete the forms and keep the NJDOE Physical Forms in their records, and will give you the new Preparticipation Form to return to the school nurse. The Preparticipation Physical Evaluation Medical Eligibility Form is due by Wednesday, October 15th.
You must also complete an NJDOE Health History form and Sports Participation form in the Genesis Parent Portal under the forms. These forms are also due by Wednesday, October 15th.
